PRENATAL CASE MANAGEMENT
The goal of the prenatal case management program is to provide the necessary services to pregnant women, which will assure positive outcomes from pregnancy. The goal is to decrease the numbers of children born with low birth weight, pre-maturity, birth defects, and lack of prenatal care.

Clients receive approximately 9-11 visits. The Public Health Nurse makes a home visit at least once a month in between scheduled physician visits for prenatal care and completes necessary teaching, counseling, case managing, and reinforcing of physician's orders. Referrals for the program are received from many sources: physicians, Family Planning, WIC, Human Services, schools, friends, county nurse, hospitals, and other counties.
